A staff member will contact you to set up the date/time to bring your child for screening. To help you be prepared for the screening, please bring copies of the following items to the screening: proof of income (current W2 or three months of paystubs for working members of the household), current immunization record, vision screening, dental screening, and physical/well child checkup. A child may qualify for the preschool program if they: -- Are four years old by August 1 and meet income requirements, -- Have a recognized disability or speech delay at age four, or -- Have a disability or speech delay at age three, even if their fourth birthday is after August 1. *COMPLETING THE APPLICATION AND SCREENING PROCESS DOES NOT GUARANTEE PLACEMENT IN THE PRESCHOOL PROGRAM.
